The state health department requires all children to have proof of immunization on file.

It is parent's responsibility to keep these forms updated and to notify the school when anything changes. Please do not send your child to school if any signs of illness are present. For the protection of all children, sick children should be kept at home. A child should be free of fever for 24 hours before returning to school.

Communicable Diseases: A child who has a communicable disease that poses a substantial threat to the health or safety of the school community may be removed from school by the principal.
